Baxter House is a twelve-bed residence in Buchanan County for adult males and females with moderate to profound mental retardation. Staff provide 24-hour supervision. Training is provided for personal skills, fine/gross motor coordination, safety skills, social and communication skills, and leisure activities. Involvement with family is encouraged. Families are included in periodic planning for resident needs. This program is funded by Medicaid as an intermediate care facility. Mountain View
Mountain View is an eight-bed residence in Buchanan County for adult males and females with moderate to profound mental retardation and/or severe behavior problems. Training is provided for personal care skills, fine/gross motor coordination, safety skills, social and communication skills, and leisure activities. Staff provide 24-hour supervision. Family involvement is encouraged and families are included in periodic planning for resident needs. This program is funded by Medicaid as an intermediate care facility. The Cottage
A three-bed residence located in Russell County which serves adult male and female individuals with developmental disabilities. The Cottage specializes in providing services to individuals with challenging behaviors. Emphasis is placed on activities of daily living and community integration. Residents play an active role in planning leisure activities and community outings. Individuals and their families participate in personalized service planning. Staff provide 24-hour care and are experienced in dealing with extremely challenging behaviors. Payment for room and board is made by the resident and is usually a percentage of the SSI or Social Security benefit. Generally, services are billed under Medicaid Waiver. Lebanon Group Home
An eight-bed residence in Russell County for adult males who are transitioning into independent living. Residents must demonstrate the ability to perform daily activities of living independently and must also demonstrate safety at home and in the community. Residents play an active role in planning the leisure activities, which may include unsupervised activities in the community. Residents and families are active in service planning. Payment for room and board is made by the resident and is usually a percentage of the SSI or Social Security benefit. Generally, services are billed under Medicaid Waiver. Elk Garden Group Home
Elk Garden Group Home is an eight-bed residence in Russell County for adult females who are transitioning into independent living. A fifteen-bed program in Tazewell County for adult males and females with moderate to mild mental retardation. Staff provide 24-hour supervision. Residents of Cumberland Mountain Home are generally more independent in their functioning than residents of Intensive Care Facilities and require assistance rather than training for all areas of independent living. Emphasis is placed on community integration. Residents and family take part in service planning.
